INSTRUCTIONS FOR COMPLETING
THE NEW MEMBERSHIP APPLICATION
In order to view or print your application, you will need Adobe's Acrobat Reader software installed on your computer. Click here if you need a free copy of Acrobat Reader . Then follow the instructions below.
1. Please print or type the information requested on the application form.
2. Fill out the complete application before submitting.
3. An email address is required. If you do not already have one, please obtain one as soon as possible
4. The section requesting you to describe your duties must be specific in relating your responsibilities in the areas of high technology security/investigations.
5. The section requesting an HTCIA membership sponsor is very important - make sure your sponsor is well acquainted with your high technology responsibilities and is a current member in good standing. Your sponsor must sign and date this section.
6. Read the HTCIA code of ethics section both on the front and back of the application.
7. Sign the application and date.
8. The fee for new membership into the Silicon Valley Chapter of the HTCIA is $50.00. The renewal fee is $40.00.
9. Annual membership dues are renewable between 1/1 and 3/31 each year.
10. Mail the completed application with your membership fee to:

12. Your application will be processed within 45 days of receipt and a letter will be forwarded to you advising of the results.
